Medical emergencies

AMH Nationwide is a screening service and not intended to take the place of your primary care physician. If you are currently experiencing a medical emergency please visit your nearest emergency room.

Jurisdiction

These Terms are to be construed and enforced according to the laws of the State of Illinois. You or your organization agrees to personal jurisdiction in the State of Illinois, and any cause of action related to these Terms of service will be brought exclusively in a court in Saint Clair, Illinois.

Waiver of jury trial

You or your organization KNOWINGLY AND VOLUNTARILY waive the right to a trial by jury in any actions related to this site.

Limitation of liability

Use of this website dictates you agree to and hold harmless, AMH Nationwide including its employees and contracted officers from liability to you, or any organization you represent from loss or damages.

Should such claims result in a liability to the company the maximum liability for all such claims and other matters will not exceed $100.
